Registration
The Surgery List is open to take in new patients in our Surgery Area.
New Patients
On joining the practice you will be offered a “New Patient Interview”. This enables us to do some simple health checks. It also means we will know about any previous medical problems you may have had, any tablets you are taking and any allergies you may have.
Not Seen in the Last 3 Years?
Patients between the ages of 16 and 75, and those aged 75 years and over who have not been seen by a Doctor or Nurse for any reason during the last three years will be offered a consultation if requested by the patient.
Not Seen in the Last Year?
Patients aged over the age of 75 who have not been seen by a Doctor or Nurse for any reason during the last year will be offered a consultation if requested by the patient.
Full Registration
If you wish to register as a patient of this Surgery, you will need to bring your medical card (complete and sign section A) into the Surgery. If you are unable to locate your medical card there is a simple form available from the Surgery to enable the registration process, this form is also available below.
To register with the surgery, all you need to do is complete a registration form and take it with you to the surgery. As it may take several weeks for your medical records to be forwarded from your previous doctor, you are required to make an appointment with the Health Care Assistant for a check on your health and to provide us with some information about yourself.

The Process
You will need to contact the surgery on 01384 277377 to arrange a New Patient Interview with our Health Care Assistant. Please bring a specimen of urine with you to this appointment. Our Health Care Assistant will undertake your height, weight, blood pressure etc and will also ask you about any previous or current illnesses you may have and also any medication you may take.
Until you have had this interview undertaken you are NOT registered with the Practice. You will need to bring your completed registration, lifestyle questionnaire and ethnicity data forms duly signed to this appointment. We will also require the following documents:
- Proof of Identity e.g. Passport/Driving Licence
- Proof of your address e.g. Household Utility Bill, Corrsepondence to you at this address
Please ensure you bring the completed forms and documents with you when you attend for your new patient interview.
Temporary Resident
If you are only in the area for a short time (more than 24 hours), you may register with the surgery as a temporary patient for up to 3 months. If you are staying for a longer period of time, you will be required to register with the surgery in the usual way.
You will need to complete a Temporary Resident form, available below and take it with you to your appointment. Please contact the surgery on 01384 277377 to make an appointment (please inform the receptionist that you are booking an appointment as a Temporary patient).
We also require proof of identity in the form of a current driving licence or current passport. Please bring this with you when you attend the Surgery.
